Please note not all archetypes clinical forms will be fully complete and some may need some exploration of intellectual property before they can be used. Over 200 archetypes clinical forms have been developed so far and these are grouped below according to clinical areas. Clicking on the individual archetype links clinical form link will take you to an individual archetype clinical form page showing various views of the structure, indication for use, screenshots and some guidance to public domain availability versus probable licensing to be explored prior to use. These archetypes clinical forms are to be considered as beta use at this stage.
